Firstly think about the purpose of the process you are assessing. What is it intended to deliver? Once you have that clear in your mind, then rate the following:
- How often is the output or deliverable (information, data, product, materials) incomplete, to the point necessary for the next group of people to perform the required sequence of steps?
- How often do the next group of people have to circle back with people in the process before, to ask for missing information or perform a task that should already have been done?
- How often is the output from the process different (quality and consistency of information) from person to person performing the task?
- How often is the information recorded missing from the location where it is retrievable by people further down stream?
- How often are there significant differences in the speed in which people perform the steps and activities to produce the process output or deliverable?

Total your score. If the result is 10 or less then your process needs to be redesigned and is costing your organization in time, and undoubtedly quality.
